- About you — your name, date of birth, contact info, and address
- Your family — spouse info (if applicable) and children 18 & under
- Ministries you'd like to serve on — pick as many as interest you
- Emergency contact — someone we can reach if needed
- Media consent — permission for your photo/video to appear in church materials
- Additional prayer / next steps — baptism, salvation, discipleship, and more
- A photo of yourself (optional) — helps our team recognize you when you visit
